Dear Parent/Guardian,

Below is a form to request that your child(ren) receive milk during their lunch at school. You may sign up for milk whether you buy lunch at school or bring your own. The cost for the ‘08-’09 school year is $40 per child. Milk payments should be paid in full by check and included with the form below. NO MILK ORDERS WILL BE ACCEPTED AFTER AUGUST 4, 2008. Milk will be available at school beginning August 25 and not August 18 (the first week of school.)

PLEASE PRINT OUT THIS FORM, FILL OUT AND RETURN TO SCHOOL IMMEDIATELY. Print as many as you need for your family.

PLEASE NOTE: Even if you are not ordering milk, check the appropriate box and return the form so we have the information on file. No credits/refunds will be issued for milk. If you have questions regarding our milk program please call me at 356-9632.
